It sounds like he may be over weight. The extra fat can hide the genitals and make it messy around there as well. Being on an unhealthy food will for sure make an animal smell worse.

Neem oil is made from the neem tree. It's great for most any skin condition, but it does stink. It has anti bacterial, anti viral, anti fungal properties and can even work for mites. When they have kidney or/and liver problems they will go up and down like that. There are some things that can be done to extend life by making them more comfortable, like phosphorus binders on the food if it's kidneys and denamarin for liver. The vet would normally prescribe them after a blood test...

It's worth trying. There's some chance that once the parasites are killed and swelling goes down in the brain there could be some improvement if the issues are from the swelling and not permanent damage.

Might be cerebral nematodiasis (brain worm disease). Pat has had to deal with that before and it presented the same as the animal unable to lift it's body or head. Unfortunately the odds of recovery are not good. Ivermectin kills the parasites but it wont reverse the brain damage.
