Hello fish-people - where are you...?

I have a real problem keeping my aquatic plants alive lately.
I've used fertilizer tablets, dinosaur piss, even mixed my own CO2 from water, sugar and yeast but my plants keep on diminishing.
Ok, ok I'm not that thorough with fertilizing but I never was, and I had great looking tanks.
Thoughts?

Do you have "plant approved" lights on your tank? Also the pH might be hurting them depending on what plants you have.
